Before You Submit
- Confirm that your items are on our accepted items list. Only items on the list qualify as donations to Furniture Assist.
- Pickup must already be arranged separately with the third-party pickup service before completing this form. This form does not notify or contact any pickup provider.
- Use the actual date your items were or will be picked up as the Donation Date — this is the date that will appear on your tax receipt.
- After submitting, you will receive a confirmation email. Your PDF tax receipt will be emailed to you once our team has processed your registration.

PDF Tax Receipt Emailed to You
Once processed, your official Furniture Assist tax receipt will be emailed to you as a PDF. Keep it for your records — it lists the items donated and the date of your donation.
