Agency Terms & Conditions
Please read this agreement carefully before completing your Agency Partner Registration
These Terms and Conditions (“Agreement”) govern the relationship between Furniture Assist Inc. (“Furniture Assist”) and organizations (“Agency Partners”) that register to participate in the Furniture Assist referral program. By completing the Agency Partner Registration form, the authorized representative of the registering organization agrees to be bound by this Agreement.
Before You Proceed
This agreement applies to all registered Agency Partners. Your organization must be a 501(c)(3) nonprofit actively serving individuals and families in need within Furniture Assist’s New Jersey service area.
Eligibility Requirements
To participate in the Furniture Assist referral program, your organization must:
- Be a registered 501(c)(3) nonprofit organization in good standing with the IRS
- Operate a program that serves individuals and families in need of household furnishings, including but not limited to those transitioning from homelessness, fleeing domestic violence, recovering from disaster, or exiting institutional care
- Be located in or serve clients within Furniture Assist’s designated service area in New Jersey
- Maintain an active social services program with qualified staff responsible for client referrals
Authorized Registration
The individual completing the Agency Partner Registration form represents and warrants that:
- They are an authorized representative of the registering organization
- They have the authority to enter into this Agreement on behalf of their organization
- All information provided during registration is accurate and complete
- They will promptly update Furniture Assist if any registration information changes
Client Referral Standards
Agency Partners agree to the following conditions governing all client referrals submitted through the Furniture Assist portal:
- Eligibility verification. Agency staff will verify that each client meets Furniture Assist’s eligibility criteria prior to submitting a referral. Referrals submitted for ineligible clients may result in suspension of portal access.
- Client consent. Referrals will only be submitted with the knowledge and consent of the client being referred. Agencies are responsible for obtaining any required client authorizations.
- Intended use. All furniture and household items provided by Furniture Assist are for the personal use of the referred client and their household. Items may not be resold, redistributed, or transferred to other individuals or organizations.
- Accuracy. All information submitted in a referral must be accurate and complete to the best of the Agency’s knowledge.
Operational Responsibilities
Agency Partners agree to:
- Submit all referrals exclusively through the approved Furniture Assist agency portal
- Adhere to Furniture Assist’s scheduling, pickup, and delivery procedures as communicated through the portal
- Ensure referred clients are informed of their scheduled appointment date, time, and any preparation requirements
- Notify Furniture Assist as promptly as possible if a scheduled appointment needs to be cancelled or rescheduled
- Maintain current contact information in the agency portal at all times
- Notify Furniture Assist promptly if your organization closes, your program ends, or your eligibility status changes
No-Show & Cancellation Policy
Furniture Assist allocates a limited number of appointment slots each week. In consideration of other clients and agencies:
- Repeated client no-shows or last-minute cancellations attributable to insufficient client preparation by the Agency may result in a formal review of the Agency’s portal access.
- Furniture Assist reserves the right to place temporary holds on referral submissions pending resolution of recurring attendance issues.
Data & Privacy
- Agency Partners are responsible for handling all client information in accordance with applicable federal, state, and local privacy laws.
- Furniture Assist will maintain confidentiality of client information shared through the referral process and will use such information solely for the purpose of fulfilling the referral.
- Agency Partners acknowledge that Furniture Assist may contact the referring Agency to verify referral details or resolve scheduling issues.
Proof of 501(c)(3) Status
Furniture Assist reserves the right to request documentation confirming your organization’s 501(c)(3) status at any time. Failure to provide requested documentation within a reasonable timeframe may result in suspension of portal access pending verification.
Suspension & Termination of Access
Furniture Assist reserves the right to suspend or permanently revoke an Agency Partner’s portal access for:
- Submission of referrals for ineligible clients
- Misuse of the portal or referral process
- Repeated no-shows or cancellations without adequate notice
- Failure to maintain 501(c)(3) status in good standing
- Violation of any provision of this Agreement
- Any conduct that compromises the integrity of Furniture Assist’s mission or operations
Furniture Assist will make reasonable efforts to notify the Agency prior to or promptly following any suspension or revocation.
Modifications to This Agreement
Furniture Assist reserves the right to update these Terms and Conditions at any time. Agency Partners will be notified of material changes via the email address on file. Continued use of the portal following notice of changes constitutes acceptance of the updated terms.
