Referral Process – Furniture Assist

How to Refer a Client

  1. Complete all fields on the referral form (revised June 2022) and email it to us. Forms with incomplete fields will be returned without an appointment date.
  2. Await appointment confirmation. We will assign an appointment date and time, sign the form, and return it to you via email. Please remember we are an all-volunteer organization, so it may take up to two weeks to receive an appointment date.
  3. Notify the client of their appointment date and time. You are responsible for informing your client. If the client cannot keep the appointment, please notify us immediately.
  4. Provide the signed form and warehouse directions to your client. They must bring the signed form to their appointment. Click here for directions and a map to our warehouse location.
  5. Review client procedures thoroughly with your client and provide them with a printed copy. Client procedures are available on the client procedures page of our website. Clients will be held to these procedures whether or not you have explained them.

Important Reminders

  • When calling or emailing about a client’s status, please provide the client’s name or initials to help us respond more quickly.
  • Client calls will be directed to you, except for calls one to two days before an appointment when time may not permit us to reach you first.
  • All emails and voicemails are accessible only by authorized Furniture Assist personnel to protect confidential information.
